It is typically made up of one complete artificial tooth installed between two crowns. Teeth on each side of the hole hold the fixed bridge in place. A bridge is a permanent treatment and cannot be removed.
The dentist will carve the two teeth adjacent to the hole left by the missing tooth so that they can each hold a crown. The replacement tooth and the two crowns will be custom-made in a single piece.
